Event feedback form template for planners and their clients

· comtev

An event feedback form should ask about the experience the respondent actually had, leave room for a specific improvement and stay short enough to complete. For a planner, separate guest feedback from the client's debrief: guests can describe arrival, seating and service, while the client can assess communication and delivery against the agreed job.

Copy the original questions below into a form or document. This page provides wording and a review process; it does not include a live survey or claim a Comtev survey-builder feature.

Copy this short guest feedback form

Start with: “Thank you for attending [event]. These questions help the planning team understand what worked and what to improve. Please answer only about your own experience.”

  1. How easy was it to find the event information you needed? Very difficult / difficult / neither / easy / very easy / not applicable.
  2. How was your arrival and check-in experience? Very poor / poor / neither / good / very good / not applicable.
  3. How easy was it to find your seat or the right part of the venue? Very difficult / difficult / neither / easy / very easy / not applicable.
  4. How was the part of the programme you attended? Very poor / poor / neither / good / very good / not applicable.
  5. What worked particularly well? [Short answer.]
  6. What is one thing the planning team should improve? [Short answer.]

Add one question about a service only if the team needs the answer. If a guest did not use a transfer or stay for dinner, give them a way to skip that question instead of forcing a rating.

Use a different debrief with the client

The client discussion can be short and direct:

  • Did the delivered work match the agreed scope? What was missing or different?
  • Were decisions and changes communicated clearly?
  • Which part of the guest-management process worked best?
  • Were there unresolved issues with vendors, payments or the handover?
  • What would you want handled differently next time?
  • What action do you need from us now, and by when?

Record the client's actual answer. A complaint is not a testimonial to rewrite, and a compliment is not permission to publish the client's name or images. Ask separately before using feedback in your planner portfolio.

Avoid questions that supply the answer

“How wonderful was our service?” asks for praise. “How was your arrival and check-in experience?” leaves room for an honest response. Also avoid combining several topics into one rating: a guest may like the food and dislike the queue.

SurveyMonkey's event survey guidance distinguishes planning surveys, RSVPs and feedback after the event. Keep the same distinction in your process: this form evaluates delivery; it is not another invitation or a request for names.

Decide whether replies are anonymous

Do not promise anonymity if the form collects names, email addresses or identifiable links. If you offer a separate contact field for follow-up, explain its purpose and make it optional where appropriate.

Keep survey access and raw responses with the relevant team. A broad client report can summarise themes without circulating every personal comment or contact detail.

Turn responses into a small action list

Review the response count and repeated themes. Keep a distinction between a guest's report, something the team has verified and a proposed improvement.

For an illustrative example, 12 replies from 100 guests are 12 replies, not the opinion of all 100. If several describe difficulty locating a table, check the seating plan handover and agree a specific change for the next event.

Use a closing table with finding, evidence, action, owner and deadline. Add unresolved items to the job handover rather than leaving them buried in a survey export.

Comtev gives planners a guest workflow and a private client view, alongside invoicing in naira. You can use records from that work in your own debrief, while preparing and managing a feedback form separately. See how it works and use the event-day checklist to collect delivery notes before the team disperses.

FAQs

What questions should an event feedback form ask?

Ask about information, arrival, seating, the relevant programme and specific improvements. Use clear response scales and let respondents skip services they did not use.

Should guests and the client receive the same form?

Usually they need different questions. Guests describe their experience, while the client can assess scope, communication, unresolved issues and the overall handover.

Can I call a feedback form anonymous?

Only if its actual setup supports that claim. Check whether it collects names, addresses or other identifiers, and explain any optional contact field used for follow-up.
